One Stop HF Clinic

For patients with suspected heart failure and a raised BNP and patients with a previous cardiac history with signs and symptoms of Heart Failure as per pathway.

Patients who have a previous diagnosis of Heart Failure and have had an ECHO in the last year can be referred to the clinic for assessment and they will not need a repeat ECHO.

If the patient has been discharged from hospital with a diagnosis of heart failure they should be referred to the clinic for review within 2 weeks of discharge. The team are hoping this will be done from the hospital and are trying to work with the wards to get this referral done.

All referrals are to be sent to the same number 01883 713779. The clinics are on the 1st and 2nd Monday of every month in Earlswood and 3rd and 4th Thursday in Oxted.

BNP Testing

If there is no previous history of heart failure/MI but a patient presents with symptoms suggestive of Heart Failure, then the new local BNP testing service should be used.

Please note that this service is being provided by an alternative pathology lab, Pathology Plus, and NOT SASH so it requires a slightly different method of ordering and collection. A paper form is required for ordering. Practice Managers have made arrangements for BNP results to be reviewed electronically on your practice software.

Practice Managers will make sure that everyone knows of the arrangements for BNP testing and are aware of the contact numbers including phlebotomists.
